In addition to the requirements shown under the general
listing procedures, the following information is required in a request
for initial acceptance of wall mountable central office copper cable
splice housings for central offices.
Item 1: Test data indicating
that the copper cable splice housing complies with an appropriate recognized
industry standard covering such product.
Item 2: Catalog designations
for each size of copper cable splice housing.
Item 3: The maximum number of
"Tip Cable Openings" that can be accommodated by each size
of copper cable splice housings.
Item 4: The maximum cable size
with respect to pair count that can be accommodated by each size of
copper cable splice housings.
Item 5: Information indicating
how the copper cable splice housings are installed, such as wall mounted,
rack mounted, etc.
Item 6: A copy of the manufacturing
specification associate with each size of the copper cable splice housings.
Item 7: A copy of the installation
practices associated with each size of the copper cable splice housings.
Item 8: Copies of quality assurance
data which is representative of several production lots and which demonstrates
the reliability of an ongoing quality assurance program.
Item 9: Information concerning
the packaging and shipment of the product to customers.

Item sj - Splice Housings and/or Splice Housings with Patch Panels
for Fiber Optic Cables
In addition to the requirements shown under the general
listing procedures, the following information is required in a request
for initial acceptance of central office splice housings and/or splice
housings with patch panels for fiber optic cable.
Item 1: Test data indicating
that the splice housing and/or splice housing with patch panels for
fiber optic cables comply with appropriate recognized industry standards
covering such products.
Item 2: Catalog designations
for each size of the splice housing and/or splice housing with patch
panels for fiber optic cables.
Item 3: Fiber optic splicing
capacity of each fiber optic splice housing using fusion and mechanical
splices.
Item 4: The maximum number of
fiber terminations of each fiber optic splice housing with patch panels.
Item 5: The type, manufacturer,
and descriptive information of the mechanical connectors to be supplied
with the fiber optic splice housing with patch panels.
Item 6: The manufacturer and
descriptive information of the fiber optic pigtails to be supplied with
the fiber optic splice housing with patch panels.
Item 7: If stub cable is provided
with the fiber optic splice housing with patch panels, a certification
that the stub cable complies with the requirements of OFNP or OFNR nonconductive
cable in accordance with Article 770 of the latest edition of the National
Electrical Code (NEC) must be supplied.
Item 8: The manufacturer and
descriptive information of the stub cable design to be supplied with
the fiber optic splice housing with patch panels, when applicable.
Item 9: A copy of the manufacturing
specification associate with each size of splice housing and/or splice
housing with patch panels for fiber optic cables.
Item 10: A copy of the installation
practices associated with each size of the copper cable splice housings.
Item 11: Copies of quality assurance
data which is representative of several production lots and which demonstrates
the reliability of an ongoing quality assurance program.
Item 12: Information concerning
the packaging and shipment of the product to customers.
Item 13: If applicable, a certification indicating that the splice
housing and/or splice housing with patch panels for fiber optic cables
are listed for their intended purpose from a nationally recognized listing
such as UL, etc.
